Case Management - Mental Health
This program is designed to assist and support individuals who are considered to have a serious mental illness.
Community support workers and the client will develop an individualized service plan to assist persons in developing the skills and supports necessary to maintain a level of independence that allows them to live and thrive in the community. Supports are provided in the community setting or within the person's home environment according to the individual's choice.
The program provides:
- assistance with daily living (i.e. housing, financial and advocacy)
- support in employment/vocational
- social activities: life and social skills development
- assistance navigating the mental health system
- health promotion and education
- crisis prevention/intervention
- developing support plans using a recovery philosophy
- referrals to other community resources.
Referrals are accepted by any source including: family; physicians; service providers; and self.
