Case Management - Mental Health

Program Description

Intake is the first point of contact for individuals requesting mental health case management or brief mental health support. This support is generally available for 1-3 months and during that time individuals can work on brief goals including referrals to other agencies, basic coping skills, and symptom management.

If an individual is requesting case management services, the Intake Worker will complete an Ontario Common Assessment of Need and together the worker and client will develop a treatment plan.

Self-referrals, referrals from physicians and other community organizations are accepted.
