Case Management - Mental Health

Program Description

Provides case management support services to people with a mental health condition, addiction, or concurrent disorder, who have been in conflict with the law. Ensures appropriate linkages with other mental health services as well as criminal justice, corrections, and social services, in order to improve service coordination and support.

Provides supportive outreach client identification, comprehensive individualized assessment and planning, systemic advocacy and coordination, and assistance with accessing and maintenance of psychiatric services.

Access Information

Referrals accepted from self, family, friends, doctors, community agencies, and other professionals, hospitals or institutions. An Ontario Health Card is not required to access this program.
