Case Management - Mental Health
Clinical Services provides service to individuals from a psychiatric rehabilitation approach. Individuals will look at their readiness for change and set overall rehabilitation goals in the environments of living, learning, working or social.
Completion of an OCAN assessment and readiness assessments and if they are not ready to set a goal, they will work with the Practitioner to develop their readiness through one-on-one sessions or group work. Individuals will identify the skills or supports needed through the Functional and Resource Assessment which will assist them in performing the skills at the required level for the goal.
Case Managers offer personal support and advocacy. The service of the program is designed to be transitional. Individuals come to the program to get the service they need in order to set goals and regain a meaningful role in their community.
An intake assessment includes a discussion of opportunities, desired outcome and possible referrals to supports and services provided internally or by other community organizations.
The intake worker is also available to assist with issues that require brief service and to provide general information and assistance to family members.
