Case Management - Mental Health
The Geriatric Psychiatry Community Service is a community outreach program offering mental health services to older adults who are unable to access the onsite Ambulatory Mental Health Clinic or who require case management.
Key services include:
- Psychiatric consultation and assessment by a Baycrest Hospital psychiatrist, in collaboration with clinical case managers
- Treatment recommendations with time-limited follow-up, including support and advocacy for the best possible services for both clients and families
- Referrals to other Baycrest services and community programs
- Psychiatric treatment, including medication management, supportive counseling, caregiver education, and access to community resources
- This program primarily serves homebound individuals or those requiring case management services.
Referrals from a physician or nurse practitioner for this service can be initiated through Baycrest Central Navigation.
The Central Navigation team will assist community agencies, primary care providers, and other healthcare professionals in navigating Ambulatory Services at Baycrest.

Inclusion criteria:
Adults over the age of 65 with a diagnosis or suspected diagnosis of depression, bipolar mood disorder, mood disorders associated with neurological conditions, mood disorders associated with bereavement, or anxiety
