Program Description

Case management services are designed to increase housing stability for people with problematic substance use and to reduce pressure on the emergency care and acute care systems.

Access Information

Referrals are accepted from all sources, including self-referrals.
Referrals are processed through the Access Point Northwest. Please complete online application form.
Individuals may call the program for assistance completing the application form.
