Assertive Community Treatment Teams (ACTT)
The Assertive Community Treatment Team (ACTT) is a team of mental health professionals that provide client-centered treatment, rehabilitation and support to individuals with severe and persistent mental illness. It is intended for individuals with the highest intensity of need due to complex, long-term serious psychiatric issues.
ACTT provides assertive outreach, taking the service to the client by providing: ongoing continuous services, 24-hour crisis support, supportive counselling, symptom assessment and management, medication education, prescription administration and monitoring, direct assistance with activities of daily living, skills teaching, development and support of social activities and relationships, substance use services, direct assistance to minimize involvement with the criminal justice system, training and support to find and maintain employment, advocacy and support, and education to family and natural support systems.
Self referrals are accepted.
Individuals are thoroughly assessed by the multidisciplinary team and must meet strict criteria for admission. Ministry of Health(MOH) criteria includes a diagnosis of Axis One disorder, a history of repeated admissions to a psychiatric facility or incarceration, homelessness or near homelessness, a history of non-compliance with recommended treatment without assertive support.
