Program Description

Community-based case management for individuals with serious mental illness and substance use issues who are in conflict with the law for minor offences.

Program includes:
- ongoing assessments
- treatment planning
- coordination of required services
- individual, group and family counselling
- conflict resolution and crisis prevention
- housing supports
- discharge planning including referral to community resources
- court diversion order coordination

Access Information

Referrals are received by the following sources: court support and diversion program, police, probation and other community organizations.
