Program Description

The Family System Navigation program provides assessment for families and individuals who need support navigating the health care and mental health systems in Ottawa. Based on the assessment, these families will be referred to appropriate services, including internal and external mental health and addictions services, primary care, housing and other programs to address the social determinants of health and promote healthy living of Indigenous families.

Access Information

Anyone can refer including self-referral. Phone or email for an appointment is preferred, walk-in service can be available.
