Case Management - Mental Health
Offers a supportive framework for individuals aged 65+, with serious mental illness to achieve greater independence and an improved quality of life. Case managers work in partnership with individuals to identify areas of strength and determine areas of need. Together, a support plan is prepared, addressing areas such as:
- skill development, particularly as they relate to employment
- issues related to accessing primary healthcare
- referrals and advocacy and access issues for housing and/or income
- assisting clients to engage in their local community reduce their experience of social isolation
Referrals accepted from all sources. Referrals through Behavioural Supports Ontario.
