Case Management - Mental Health

Program Description

Provides services and supports for those who are surviving homelessness in York Region and South Simcoe. Access to a housing specialist to support individuals in connecting with potential landlords to secure safe and affordable housing. Access to a Case Manager to work one on one on recovery-focused goals.

Inclusion criteria: Individuals experiencing chronic homelessness and/or mental health challenges Individuals and families that are homeless and in need of a rental subsidy to maintain a safe and affordable living environment Individuals and families in shelters who are in need of connection to mental health services

Access Information

Referrals are welcome from all sources. Referral is through Streamlined Access.
