Case Management - Mental Health

Program Description

Supports people who are being discharged from hospital to connect with the supports they need to stay safely in the community. The program assists peoples to stay out of hospital. In order to be considered for the Community Transition Team, the individual must be living in York Region/South Simcoe and have had attended a hospital within the past two months for a mental health concern.

Working with community partners, the team helps clients to: Transition to the community after being discharged; Access immediate support to prevent a visit to the emergency department; Develop a wellness plan; Find housing; Get support for mental health and substance use issues; and Access Peer Support. The Community Transitions Team works in partnership with local hospitals, emergency shelters, and other agencies.

Access Information

Referrals are accepted from hospitals, emergency shelters, shelters that support women who have experienced violence, domiciliary hostels, agencies providing crisis beds, and other community agencies.
