Case Management - Mental Health
Offers open-ended case management support to individuals. The individual directs the nature of their support, which may include: assessment of needs, goal development, skills teaching, advocacy, crisis intervention, recovery group, and crisis prevention.
Referral is through Streamlined Access. Please call or the application form is available online.
After calling or applying online, an access worker will contact with the individual to establish eligibility and provide information about supports and services available.
