Program Description

The program utilizes a peer support model to encourage natural support networks and include clients in the decision-making process. Case management services are offered to individuals in various residential and community settings.

Access Information

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
