Case Management - Mental Health
The program utilizes a peer support model to encourage natural support networks and include clients in the decision making process. Case management services are offered to individuals in various community settings.
Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
