Case Management - Mental Health
The Intensive Case Management (ICM) program provides comprehensive, individualized support to adults living with a mental illness or serious mental health concern. The program includes key functions such as outreach and identifying individuals who need support, thorough assessment and care planning, direct service provision, and targeted interventions. Case managers offer continuous monitoring and evaluation, adjusting plans to ensure that services remain effective and meet the individual’s changing needs. Ongoing follow-up, advocacy, and liaison services are also central to the program, helping individuals stay connected to essential resources and providing assistance in overcoming barriers. Through collaboration and consultation with other service providers and community organizations, the program ensures a coordinated, holistic approach to care. The primary goal is to help individuals function in the least restrictive and most natural environment possible while improving their quality of life. Case managers maintain an active, long-term role, supporting individuals as their needs evolve and ensuring continuity of care across different service settings.
Referrals are accepted from self, families, friends, other organizations, and the medical community.
