Counselling and Treatment - Mental Health
This program will provide individuals and/or families with healthy coping and stabilization techniques, psycho‐education and a therapeutic environment that will allow them to process unresolved trauma issues.
The program foundation will be rooted in Anishinaabe culture, ceremony and land based teachings.
The program is designed to provide support to individuals and/or families prior to treatment, in the treatment orientation phase, treatment phase and after‐care phase. This 3 phase approach will allow the program staff to provide on‐going support to individuals and community care service providers for a period of 6 months.
Program participants are expected to actively participate in all elements of the program.
Referral Sources will complete a Brief Referral for Eligibility Screening and submit it to the WFS – FHP Intake Coordinator.
The Referral Source and individual(s) will then be required to submit the complete Referral Package and required supporting documentation. Once the complete package is received, the treatment planning will begin and the Intake Coordinator will set an intake date for the family. Prior to intake a meeting will take place to answer any further questions or address concerns; review and complete the treatment plan and consents and agreements will be completed and signed. This meeting will involve the Intake Coordinator, Family, and Community Worker/Referral Source.
