Program Description

Provides clinical case management to persons with a wide range of serious and persistent mental health problems such as depression, psychosis, post-traumatic stress, anxiety or panic.

Access Information

Referrals by physicians, hospitals and community agencies are preferred; however, self-referrals are also accepted. All referrals are reviewed and priority is given to the most serious and critical referrals. Referral forms are available from the centre. Please call for information.
