Program Description

The Opiate Case Manager/Therapist provides clinical care to assist those individuals who identify the need for support to address opiate addiction for oneself or a family member.

The service provides counselling, case management, and coordination with primary care services to address client needs.

Access Information

Referrals for service are accepted from all avenues, including self, family member or friend, physician, other community services, etc.

Service provision is voluntary and designed to involve and work directly with clients to achieve the best, most effective and efficient delivery of service.
