Program Description

This program coordinates access to mental health services for adults with concurrent disorder, which is defined as co-occurring substance use and mental health issues.

The case manager works with individuals, mental health workers, and other organizations to navigate the substance use and mental health systems and provides a link between the service system, support system and care plans.
