Case Management - Mental Health

Program Description

This program coordinates access to mental health services for adults involved with, or at risk of involvement with, the criminal justice system.

The case manager works with clients, mental health workers, and other organizations to navigate the criminal justice system and mental health system, and provides a link between service system, support system and care plans.

Access Information

Individuals may be referred by outside organizations or may self-refer.
