Assertive Community Treatment Teams (ACTT)

Program Description

Community based program providing Assertive Community Treatment (ACT) services to individuals diagnosed with serious and persistent mental illness who would otherwise rely heavily on inpatient and other support services. Primary diagnosis of long term mental illness such as schizophrenia, schizoaffective disorder or bi-polar disorder. Model of service is psychosocial rehabilitation.

Referrals are received from primary care providers, individuals, caregivers and families through AccessMHA who will then assess for eligibility.
