Case Management - Mental Health
The program offers community-based mental health case management services for individuals who are in conflict with the law. These services may include accompanying clients to obtain legal aid, health cards, housing, etc., assisting clients with activities of daily living and escorting clients to medical appointments and rehabilitative programs. The case manager may also monitor and encourage the clients' compliance with treatment measures.
Service care plans are developed and negotiated with the client. The case manager does not assist the client directly with his or her court matter, but provides linkage and support to clients who require "hands on" assistance in the community. The case manager remains involved with the client until the client is linked to other services, for as long as assistance is required, or until client is no longer involved with the criminal justice system (i.e., probation or diversion has ended).
Individuals may self-refer, and should call for an appointment. Third party referrals from mental health workers, courts, and others are also accepted.
To access this program, client must be in conflict with criminal law and suffer from serious mental illness.
