Program Description

This program offers a virtual day hospital program which provides nutritional rehabilitiaon, psychological counselling, and group based services.

On-site individual appointments may be required.

Access Information

Referral only accepted from primary care provider MD (Doctor of Medicine) or Nurse Practitioner. Please complete and fax the referral form found online.

Metabolic monitoring by a General Practitioner (GP) is required while individual is waiting for the assessment and services. Individual can still be referred if they do not have a GP. Individuals must receive treatment for drug use issues prior to entering program.
