Case Management - Mental Health
Wraparound is a case management process that helps families with complex needs function more effectively in their homes and communities by supporting the youth through an integrated plan. The plan is designed by the youth and their team to help them achieve their vision and focuses on strengths, needs, and culture. Wraparound is designed to teach families the skills and confidence to be independent. Clients have needs identified in two or more life domains (i.e., housing, social, emotional/psychological, educational/vocational, safety, medical). In most cases, clients referred to Wraparound would have experienced multiple treatment breakdowns and have received services from many service providers in the past.
Wraparound is a voluntary program therefore the identified client needs to consent to service.
Families can be referred to Wraparound following a Choice appointment. Professionals working with the family can also refer to Wraparound by completing the Wraparound Referral form.
Youth are excluded if they are in the care of the Children’s Aid Society or already receiving case management support from another program.
