Case Management - Mental Health

Program Description

Short-term intensive case management services for individuals with mental health and addictions issues who are being released from correctional custody. This includes linking clients to longer-term case management services, rehabilitative programs/medical treatment, safe beds, court support services, and accessing appropriate housing and financial assistance. The program also provides support and information to family members and/or significant others.

Access Information
