The Child & Youth Mental Health Outreach Case Manager provides case management supports for children and youth navigating the child welfare system, mental health, and other health and wellness services, bridging the transition between youth and adult services.

Self-referrals are preferred, and referrals from organizations, professionals, and family members are also accepted.
