Counselling and Treatment - Mental Health

Program Description

Provides non-emergency assessment, consultation and treatment, with care provided by Registered Nurses in collaboration with a geriatric psychiatrist or geriatrician. Services may include cognitive testing, supportive counselling, caregiver education and planning for ongoing care.

Access Information

This service is available to individuals aged 65 and older, as well as individuals under 65 who have a diagnosis of dementia or an intellectual disability with cognitive or behavioural changes. The service can be accessed through referrals submitted through the Regional Geriatric Program at St. Joseph’s Healthcare Hamilton or directly to South Coast Wellness.
Referrals may be made by individuals, practitioners, community care providers, guardians or substitute decision-makers by phone, in person or through the online referral form on the website.
While a physician referral is not required, lab tests may be required early in the process.
