Assertive Community Treatment Teams (ACTT)

Program Description

The ACT Team is a community-based, outpatient treatment program providing services to individuals with a serious mental illness. Services are provided in the individual's environment and include monitoring, assessment, and teaching about medications as well as housing assistance, vocational support, recreational support, life skills teaching.

Referrals are accepted from mental health care facilities, community providers, and family physicians.
