Program Description

Geriatric psychiatrist and behavioural support outreach nurse provide consultation and follow-up psychiatric care to individuals living in the community. Support and consultation are provided in collaboration with the individual's physician.

Access Information

Referrals only accepted from physicians of long-term care facilities of Lanark County.
Referrals are mailed or faxed to Intake Secretary -Geriatric Psychiatry.
Outreach Registered Nurses triage referrals, and liaise with long-term care facility and community partners where applicable.
