Assertive Community Treatment Teams (ACTT)

Program Description

A multidisciplinary team that serves individuals experiencing serious mental health concerns combined with aging-related complexities. The service includes social work, nursing, occupational therapy, behaviour therapy, geriatrics and geriatric psychiatry.

Eligibility Criteria: To be eligible an individual must be 65 years or over (with some exceptions for those 55+), living in the Home and Community Care Support Services Toronto Central Region boundaries (roughly Islington Ave to Warden Ave, Eglinton Ave /401 to the Lake) with priority given to those living west of Yonge St, east of Kipling Ave., south of Eglinton to the Lakeshore.

The individual must be experiencing serious mental health problems, be having difficulty living in the community, have an inability to consistently manage their daily living activities or to maintain a safe living situation, and have had multiple recent hospitalizations or use of hospital emergency departments.
