Program Description

The Assertive Community Treatment (ACT) Team provides intensive community supports to individuals experiencing serious mental illness and their families.

Access Information

Anyone can refer. Call, email, or see website for registration form.

Psychiatrist assessment will determine eligibility. Intake assessment by intake worker, psychiatrist and program manager, along with client and significant others for admission.
