Program Description

The Transitional Aged Youth (TAY) Clinic is designated for youth who, due to having major mental illness, are at risk of harm to self or others, or at high risk of having major impairments in functioning and meeting developmental milestones as they transition into adulthood.

Referrals are accepted from primary care physicians or nurse practitioners.
