Program Description

Addresses the health and social service needs of hard to reach population with complex needs. Supports residents in Durham Region who are homeless or at risk of becoming homeless and who may be dealing with mental health and addiction challenges and/or may require medical care.

Provides primary care treatment, mental health counselling, crisis intervention, navigation support, and assessment developing follow-up plans as needed. Follow-up may include brief monitoring/support, referral to appropriate community organizations/support networks, or collaboration with existing supportive organizations.
