Program Description

The community support worker program provides practical assistance and support to clients and their families with the aim of filling existing service gaps within the community.

These services may include (but are not limited to) the following:
- practical in-home assistance
- referrals to community services
- assistance with technology and assistive devices
- client advocacy
- assistance while navigating government services

Community support workers will provide these services via the following methods:
- home visits
- video conferencing
- one on one
- accompanying clients to appointments
