Program Description

For individuals with a mild to moderate mental health condition. Includes assessment & diagnosis, treatment recommendations, medication management with follow up, and counselling.

Access Information

Physician's referral required. Patient must not be affiliated with an existing Family Health Team or Community Health Centre (CHC).
Solo practicing physicians in Windsor can refer into this program once the physician is affiliated and oriented to the model of care at the Centre. Please call for information.
