Transitional Discharge Model Program is conducted within Belleville General Hospital, Hotel Dieu, Providence Care, Kingston Health Sciences and Brockville Hospitals. It is a program designed to help individuals being discharged from hospital, transition back into the community,

Access Information

Anyone can refer including self referrals. Referrals can be through AccessMHA Coordinated Access Service or through the organization directly. Utilize AccessMHA.ca website or call for intake if accessing the website is not accessible to the client.

Individual must be currently be in hospital to receive this service.
