Case Management - Mental Health

Program Description

Offers rapid case management supports to residents of northwest Toronto to access mental health and social services in their immediate communities. It is open to anyone ages 13+ who are experiencing or are at risk of negative mental health outcomes.

Access Information

Referrals accepted from all sources, including self-referrals.
Please call, email or complete the online application form.

Program serves residents of northwest Toronto, ages 13 and older, who are at risk of negative mental health outcomes or are experiencing mental health challenges.

Program is also available at The HUB@2115. 2115 Finch Avenue W, Suite 204 - Building C.
