Program Description

The program helps Francophones make the transition from child to adult mental health services. The content of the group is adapted to the participants’ diagnosis, even more so, to their objectives. The program offers a referral service throughout the program to make it easier to obtain treatment tailored to the needs of each participant.

Le programme a pour objectif d’aider les francophones à faire la transition entre les services en santé mentale pour jeunes et les services pour adultes. Le contenu du groupe s’adapte aux diagnostics mais surtout aux objectifs des participants. Le programme offert un service d’aiguillage tout au long du programme, pour faciliter l’obtention d’un traitement correspondant aux besoins de chaque participant.

Access Information

Completed referral by psychiatrist a family doctor or nurse practitioner sent to central intake.

French to follow:
Référence complétée d’un psychiatre, médecin de famille ou infirmier praticien acheminée à l’accueil centralisé.
