Program Description

Transitional case management (TCM) is a short-term, community-based program, designed to help navigate mental health concerns, community supports, connect to resources and services and meet an individual's short-term goals. Services are provided to those individuals with mental health issues in Exeter and South Huron County and are brief in nature lasting approximately 4-6 months.
