Program Description

The Case Management and Service Coordination program offers one-to-one support for children, youth and families who require support accessing and/or navigating services, particularly when complex needs exist and/or multiple services are being accessed.

This program also offers access to psychiatrist or psychologist for assessment and follow-up.
