Community-Based Withdrawal Management
The teams include outreach counsellors, a social worker, a nurse and a peer support specialist. A nurse practitioner sees each client.
Clients receive two points of contact from their care team a day either by in-person appointments or virtually. Our team works with clients in a trauma-informed, harm reduction based approach offering a balance of Indigenous and Western approaches to help clients work toward their goals.
We accept self-referrals and referrals from community and health service providers. All requests for the program are connected through central referral which is supported by Mamaway Wiidokdaadwin.
Each individual must identify a support person who is available to help them as they participate in the service. The support person must be 18 or older and available for at least the first 5-7 days of participation.
