Program Description

A collaborative community-based program offered to residents living at 4175 and 4205 Lawrence Ave E. Scarborough Centre for Healthy Communities has partnered with Fred Victor, Cota and Toronto Community Housing to support the community with the following team: Community Outreach Team, Case Manager (COTA), Case Manager (Fred Victor), Housing Coordinator, Peer Support Worker, Independent Support Worker, Registered Nurse and CSS Intake and Program Support Assistant.

Harm reduction supplies and access to primary care are also available onsite.

Access Information

Self-referral or via community organizations, primary care providers and other stakeholders. Residents of 4175 or 4205 can drop by the LEPP offices, attend programs or contact program assistant through telephone/email to get connected.

Some programming and outreach occurs in evening hours. LEPP team members can provide flexible supports to attend appointments outside of regular hours if needs be.
