Support Within Housing - Mental Health
Provides supportive housing for people diagnosed with a mental health condition and/or addictions issue, who are homeless or at risk of homelessness.
The goals are: to assist people to live as independently as possible; to assist people to participate in the community; to provide supports to minimize stress, manage illness and prevent crises and hospitalization; and to provide education and support to enable people to develop and maintain skills and relationships.
Individuals are to be referred though shelter services, homeless outreach programs or homelessness providers to the Coordinated Access System in Peterborough and be placed on the By-Name Priority List (BNPL).
The Service Prioritization Decision Assistance Tool (SPDAT) must be completed by one of the above services.
