Program Description

Post-Court Transitional Case Management aims to assist individuals who are completing their court matters and live with a mental health and/or substance use concern. Goals are identified and worked on through service coordination, care planning, skill development and supportive counselling to promote recovery.

Self-referrals and referrals from caregivers can be made by calling the Intake Coordinator or by email.
