Assertive Community Treatment Teams (ACTT)

Program Description

Flexible Assertive Treatment Teams (FACTT)provides a flexible, recovery-oriented model of care for individuals with complex mental health and addiction needs. Care and treatment can be “flexed” based on the needs of the individual.

Focused on:
- personal goal and recovery planning
- supportive and therapeutic counselling
- crisis intervention and crisis management
- service coordination and health system navigation
- assist in acquiring and maintaining housing
- guidance in financial assistance and money management
- health monitoring via FACT team nurses (i.e. injections, diabetes, medication delivery)
- life skills and relationship building
- psychiatric care

Access Information

Referrals to this program are made through Here 24/7 and they are brought to the Community Triage Group which meets weekly to review individuals that are referred by Here 24/7. The community intake process is made up of mental health service providers within our region.

Access to the program is based on the FACT standards guidelines.
