Program Description

The Harm Reduction Case Management program provides support to individuals who are looking to make changes with their substance use. Case managers will engage individuals and will support individuals with system navigation and referrals to other services. Services are based on an assessment of the individual's needs.

Self-referral accepted. Walk in to speak with a case manager. No appointment required and services are based on "in the moment support."
