Program Description

Coordinated Access offers comprehensive assessments for individuals experiencing mental health issues and requiring access to services. Individuals and their loved ones may then be linked to a wide variety of services depending on need within CMHA-Windsor Essex, Hotel-Dieu Grace Healthcare, or the community.

Access Information

Anyone can refer including self-referrals. Call, fax, fill out online referral form, or walk in at the Crisis & Mental Wellness Centre to speak with a Coordinated Access worker.
