Case Management - Mental Health

Program Description

This service provides specialized assessment, support and case management services to individuals who have a serious mental illness (and their families) who are 60 or older. Health teaching, counselling, and support is provided by a multidisciplinary team consisting of psychiatric nurses, social workers, and mental health case workers with specific training in psycho-geriatric issues. Specialized group for seniors with depression provided.
