Program Description

The Supportive Housing and Health Initiative (SHHI) is a comprehensive program, designed to assist tenants residing in both fixed and scattered supportive housing sites. The initiative aims to provide a holistic approach to care, recognizing the significant link between housing and health outcomes. The program is specifically tailored to support individuals transitioning from chronic homelessness to supportive housing, a change that, while beneficial, can present numerous challenges.

The main goal of the SHHI is to facilitate tenants' successful adaptation and thriving in their new living environments, thereby promoting recovery from homelessness and improving overall well-being. By integrating the SHHI team into supportive housing sites and existing support frameworks, the initiative ensures that tenants have seamless access to the health expertise and support necessary for their journey toward recovery and stability.
