Program Description

Centralized Intake provides a single point of contact for referents and community members to access immediate supports and brief services across the province of Ontario.

​Centralized Intake supports the needs of community members provincially using coordinated access and response approach.

- allows community members a single point of entry to services offered across the province
- provides immediate, short-term, focused support services
- alleviates wait lists
- assesses community member needs to support internal and external referrals to various programs and services
- safety planning and advocacy supports
- connections to education, information, and community resources
- cultural supports and holistic services
